This educational program provides a comprehensive overview of hospice care, designed to inform patients, families, and caregivers about its purpose, processes, and benefits. It covers what hospice care entails, eligibility criteria, steps to initiate hospice services, common misconceptions, and what to expect during the hospice journey. Through clear explanations and practical insights, this program aims to empower participants with the knowledge to make informed decisions about end-of-life care.

 

Learning Objective:

Define Hospice Care: Understanding the core principals and purpose of hospice care, including its focus on comfort and quality of life.

Identify Eligibility Criteria: Learn the medical and regulatory requirements for hospice eligibility, including terminal illness certification

Outline the Process to Start Hospice: Gain knowledge of the steps involved in initiating hospice care, including referrals, assessments, and care planning

Dispel Common Myths: Addresses and clarify misconceptions about hospice care, such as it’s scope, timing, and impact on patients and families

Understand What to Expect: Explore the typical experiences, services, and support provided during hospice care to prepare patients and families for the journey
